New Patients

Your first visit may last up to an hour and a half, depending on the issues you wish to discuss with Dr. Coshow.  If you have chronic health concerns it is very important that we have your previous medical records and an accurate list of your current medications and supplements, including the dosages, at the time of your first appointment.

New patient forms need to be completed before your first appointment.  Please bring them to your appointment, along with your insurance card, medical records and list of medications and supplements. You can download the forms using the links below.

 

Accepted InsuranceRegence Blue Shield, Premera Blue Cross, Lifewise Health Plan of Washington, First Choice Healthcare Network, Bridgespan, Kaiser Access PPO, Auto Injury Insurance carriers, Labor and Industries and HSA accounts.   All plans offer differing coverage for naturopathic medicine.  Check with your insurance to be certain you understand your coverage.  

Dr. Coshow is not a preferred provider with Kaiser HMO, Aetna, Cigna, United Healthcare, Medicare, Medicaid, Molina, Coordinated Care or Apple Health. 

Billing out of network:

We do not bill out of network benefits, as it often results in a high financial cost to the client and inconsistent payout to the provider. If you have out of network benefits that you would like to use, we can provide an itemized receipt that you can submit directly to your insurance to request out of network reimbursement.

Patients who pay at the time of the visit are given a discounted rate.  The fee schedule can be viewed above.
